Costume Order Form
IMPORTANT | Please read:
1) This online form is due Friday, December 15th, 2023. A $50 late fee per dancer applies to all forms received after this date, and costume availability is not guaranteed after this date. Forms will not be accepted after December 31st.
2) Costume measurements will be taken by you and submitted to us on this form. Instructions can be found below. We recommend checking the box to “size up” if you anticipate your child will grow between now and June 2024.
3) If you choose to pay by check or with a different credit card than the one on file, indicate your payment preference on this form. All payment MUST be received by December 15th, 2023 to avoid a $50 per dancer late charge.
4) Payment information and the measurements of each dancer must be included with every form received.
Please Fill out the below information in its entirety, and submit one form per performing student. Each class requires only one costume unless otherwise notified.
*Company dancers: please do not include the Company dance piece in the below calculation.
